              AUTHORIZATION FOR SCA COMBAT RELATED ACTIVITIES

Anachronism, Incorporated, in the following activities (signature and initials

The above listed persons certify that they are duly authorized and warrented 
marshals designated by the Earl Marshal or his representative as able to 
authorize individuals in the above activities.
                                              
The above listed marshals certify that, if the person being authorized for an 
activity is a minor, the parent or legal guardian of  the minor is famillar 
with the activity and approves of their child's participation, and has 
signified that fact by executing the Parent's Authorization Statement, to be 
filed along with this authorization.

TO THE PERSON BEING AUTHORIZED: You must have this form filled out and signed 
by a duly warranted and authorized marshal. It need not be executed at an SCA 
event. If you are a minor, your parent or legal guardian must have executed a 
Parent's Authorization and Consent for SCA Combat-Related Activities form.

This Authorization form, a duly executed Waiver and Informed Consent form, 
and(if you are a minor) a Parent's Consent form must be presented to the 
authorizing marshal for you to recieve to recieve an Authorization Card. If 
you already have an Authorization Card for other activities, you do not need 
to re-execute this Authorization form. The authorizing marshal will date and 
initial the appropriate area on your card, update your Authorization form and 
inform the Kingdom Minister of the list of the additional authorization.
