I certify that I have witnessed each activity listed above, understand its 
implications, and approve of said minor's participation in this activity at 
SCA events. I certify that I have executed a MINOR'S WAIVER and INFORMED 
CONSENT to PARTICIPATE in SCA COMBAT-RELATED ACTIVITIES form, to be filed with 
the proper SCA office along with this PARENT'S AUTHORIZATION STATEMENT and the 
AUTHORIZATION for sca COMBAT-RELATED ACTIVITIES form.

No minor may be authorized to participate in any form of SCA combat-related 
activity unless the minor's parent or legal guardian has witnessed the 
activity, understands the activity and its potential dangers, and signs this 
form. The marshal who assists the parent or guardian to witness and understand 
the activity also signs the form as witness. This need not be the same marshal 
as the marshal who authorizes the minor.
This authorization form, a duly-executed MINOR'S WAIVER and INFORMED CONSENT 
form, and the actual AUTHORIZATION for SCA COMBAT-RELATED ACTIVITIES form must 
be presented to the authorizing marshal in order to recieve an AUTHORIZATION 
CARD. If you already have an authorization card for other activities you do 
not need to re-execute the minor's waiver. The authorizing marshal will date 
and initial the appropriate area on your card and inform the Kingdom Minister 
of the List of the additional authorization.
